Investigation prior to thyroglossal duct cyst excision

چکیده
I enjoyed reading the authors’ report of a computerised questionnaire evaluation of the approach of ear, nose and throat surgeons to the pre-operative assessment of a patient with a clinically diagnosed thyroglossal duct cyst (TDC). With a response rate of 64%, the majority (95%) of the surgeons contacted would arrange an ultrasound (US) scan of the neck, with a moderate-sized minority (32%) requesting thyroid function tests (TFTs). While advocating the value and cost-effectiveness of TFTs to exclude an ectopic thyroid, the authors state in their discussion that ‘Neck ultrasonography can accurately identify normal thyroid tissue in the presence of TDC’. Unfortunately this statement is not correct. In cases in which the thyroid has not completely descended, the subsequent configuration of the infrahyoid muscles may mimic the US appearance of a normally descended thyroid gland.1 While this error may be avoided by high-resolution parasagittal imaging performed by an experienced sonologist with knowledge of the clinical context, this may not always pertain, with radiological attention perhaps diverted by the presumed TDC.1 Although I would not disagree with the authors’ conclusions in relation to the results of their questionnaire study, readers should at least be aware of the possibility of an erroneous US report, suggesting the presence of a normally located thyroid gland when this is not the case.

Papillary carcinoma in correlation to thyroidal duct cyst: A case series
Background: Persistent thyroglossal duct leads to a fibrous cyst formation named thyroglossal cyst which is the most common form of congenital cyst and usually located in the midline neck below the hyoid bone. Rarely the thyroglossal cyst is associated with thyroid cancer. Although the Sistrunk procedure is often considered adequate, currently there is no clear consensus on the optimal manageme...
متن کاملPapillary thyroid carcinoma arising from thyroglossal duct cyst- A case report
Carcinoma which arises in the thyroglossal duct cyst is a very rare. About 215 cases have been reported worldwide till yet. We wish to report one such case of papillary carcinoma of the thyroglossal duct cyst in a 50 year old female. In a majority of the cases, the diagnosis is made on histopathological evaluation after the excision of the clinically benign thyroglossal duct cyst.
متن کاملIsolated Papillary Thyroid Carcinoma in Thyroglossal Duct Cyst
Thyroglossal duct cysts are the most frequently encountered congenital neck masses having a malignancy rate of 1%. The most common malignant form is papillary thyroid carcinoma. Synchronous papillary thyroid carcinoma developing in the thyroglossal duct cyst is rarely seen. Synchronized papillary carcinoma can be described as a multifocal tumor or a metastatic lesion on a thyroglossal duct cyst...
متن کاملEctopic thyroid in presumed thyroglossal duct cyst.
We report a case of an eight year old female child with a midline neck mass for seven years. Since its early presentation, this midline mass has recently increased in size. A presumptive diagnosis of thyroglossal duct cyst was made clinically and planned for surgery. Preoperative evaluation revealed hypothyroidism, hence she was referred to us for the management of the same prior to excision. O...
متن کاملSquamous cell carcinoma arising from a thyroglossal duct cyst: A case report and review of the literature
Objectives Squamous cell carcinoma in a thyroglossal duct cyst is exceedingly rare with only 26 reported cases in the literature so far, which only account for 6% of the patients. Methods We report a unique case of squamous cell carcinoma arising from a thyroglossal duct cyst in a 49-year-old male who was primarily diagnosed as a thyroglossal duct cyst with inflammation. The patient underwent...
متن کاملThyroglossal Duct Cyst Papillary Carcinoma
Thyroglossal duct cysts are usually located in the midline of the neck. The coexistence of carcinomas in thyroglossal duct cysts is extremely rare, with most being papillary carcinomas. Usually, the diagnosis is only made postoperatively after excision of the cyst. Although the Sistrunk procedure is often regarded as adequate, controversies exist concerning the need for thyroidectomy depending ...
